The long-term problem found was that lack of accessibility to the new software system the front desk administration desk has. The lack of access limits the alerts given to patients about their upcoming appointments. Short Term Opportunities When looking at the two short term opportunities for improvement, I started my root cause analysis with the 5 whys. ProblemStatement: Patient arrives to visit unable to leave urine sample. Why: Arrived with no water Why: Heavy traffic on the route to the clinic Why: No additional time to stop and grab water Why: Forgot to grab water bottle before leaving home Why: Woke up and left late for appointment Root Cause Analysis: Offer water within the waiting rooms so patients are able to drink and leave urine sample for appointment. This problem is a frequent occurrence within OB/GYN clinics, due to the importance of urine tests during pregnancy and certain procedures. With the urine sample the clinical staff will run a test to determine if any infections are present and if your kidneys are working correctly ("Why Are Patients Asked for Urine Samples?", 2020).
