Most healthcare professionals do not want to be at the bunt of causing harm to their patients. But often, the one thing that impedes upon patient safety is preventable medication errors. It is estimated that 1.5 million people per year are affected by medication errors (da Silva & Krishnamurthy, 2016). Unfortunately, many cases either go unnoticed or unreported, this number does not represent those errors. From research conducted from the previous assessment it is found that human error has a direct connection to medication errors. Not only as individuals, but as a healthcare system how can these errors be prevented? Elements of the Problem/Issue Medication errors can occur at any point of the process from order entry to administering medication to the patient. All these steps require some form of human process and leave room for mistakes to happen. In many research studies, it is shown that majority of these medication errors occur at administration. According to one study, which directly observed medication administration, 96% out of 225 doses violated patient safety (Wondmieneh et al., 2020).
