The deliberate organization of patient care activities and the sharing of information regarding patient care among all participants in the care team helps to meet the needs of the patients and achieve safe and effective care. A 2012 statement from the American Nurses Association states that “Care coordination is a necessary foundation to achieving the “triple aim” of health reform – improved patient experience of care (quality, access and reliability), improved population health, and per capita cost control” (ANA Congress on Nursing Practice and Economics, 2012, p. 1). Patients needs and preferences are the priority in care coordination.  Care coordination can be achieved by using a bread approach or using specific care coordination activities (Agency  for Healthcare Research and Quality, n.d.). The broad approach includes teamwork, care management, medication management, health information technology, and patient-centered medical home. Establishing accountability and agreeing on responsibility, communicating/sharing knowledge, helping with transitions of care, assessing patient needs and goals, creating a proactive care plan, monitoring and follow-up, including responding to changes in patients' needs, supporting patients' self-management goals, linkin
