As a member of this healthcare organization, it is important to me that our patients are provided with safe quality care and that starts the minute they enter our doors, but it does not stop once our patients discharge. Effective, efficient, high quality healthcare continues into discharge and ensures that patients have all the necessary tools to manage their care once they transition from our care to either a skilled nursing facility, rehabilitation facility or home. Wherever our patients discharge there should be continuity of care and a plan in place to help reduce readmissions and to help ensure higher quality of life. A key element in providing this is care coordination. During this presentation I will address some key elements of care coordination and what affects how care coordination is delivered.  What is Care Coordination  Care coordination is a vital aspect of providing safe quality care; however, it is not a concept that most nurses are well versed on.
