Received Gardasil vaccine at the age 14. Tetanus upon high school entry in 2014. Flu vaccine is not indicated as it is out of season. Family history includes mother and father alive and well. Mother has hypertension and father is a smoker. There is a second hand smoke exposure. Lives with parents in a rural community, is looking forward to graduation from high school this month has enlisted in Army National Guard, and leaves for basic training one month after graduations. Enjoys sports, especially basketball and remains physically active. Patient denies use of tobacco products, alcohol, or recreational drugs. Review of systems negative for any concerns. Patient is alert and oriented, cooperative, well groomed, slender, and physically fit. Vital sign are within normal range.
