Application of the RCA process to the scenario is as follows. 1.(What Happened) – A patient named Mr. B enters the ED with a fall injury and complaints of pain in his hip and leg area. The patient is assessed by Nurse J and the physician and a treatment plan is created. Mr. B is given pain and sedation medications via IV push so that the physician can perform a reduction of the patients hip. The pain and sedation meds are not working as the doctor had hoped so he orders Nurse J to give more doses via IV push. After a review of the patients chart, it is determined that Mr. B may not have responded to the initial amount of meds given because of his history of taking oxycodone and his weight. The procedure is performed successfully after Mr. B has been sedated. The ED then receives notice that a patient will be arriving via EMS that is in acute respiratory distress. Nurse J places Mr. B on continuous blood pressure monitoring and a pulse oximeter while Mr. B appears to rest comfortably. Nurse J then leaves the room and allows Mr. Bs son to sit in the room. Mr. Bs blood pressure and O2 are stable when Nurse J leaves the room. Nurse J and the LPN then heads to care for the patient who has just arrived via EMS. While Nurse J and the LPN are busy with the EMS patient, Mr. Bs pulse ox alarm starts sounding because his oxygen saturation levels are deteriorating.
