Based on community assessment data, a critical community nursing diagnosis for increased infant mortality rate related to prematurity, low birth weight and birth defects was identified and goals were developed while considering primary, secondary and tertiary levels of prevention. A community health nurse would collaborate with health care professionals that focus on woman and infant health. Along with this group, social workers and contacts with the Department of Health would be included. This partnership of professionals develop and activate goals, devise interventions and strategies, and evaluate and determine the efficacy of the intervention in achieving established goals. Planning of interventions is a multistep process which includes determining intervention level systems such as group or community, centering on primary, secondary and tertiary levels of prevention, and validating the practicality of those intervention based on available resources (Nies & McEwen, 2015). As part of those resources, the nurse would consider who can be involved in the community to make the planned interventions occur. There would be a collected effort amongst the health care professionals in plan development to evaluate the expected outcomes to determine the efficacy of implemented strategies based on set goal
