Specific Factors Leading to the Medication Error Patient Safety Event. In this scenario, the safety event includes a new graduate nurse on orientation in the ICU, his preceptor, the charge nurse, pharmacist, and the patient. The event takes place at a small Catholic community hospital whose mission is to serve those in need, especially the poor, dying and underserved. Because of funding issues, the staffing at this hospital is dangerously low. ICU nurses are tripled, including the charge nurse. The ICU doctors are only locum, there are no nursing care partners (or CNAs) and pharmacy has only 4 employees to serve the hospital™s 110 beds. Leading up to this medication error event, an MRT (medical response team) or RRT (rapid response team) was called on the patient on a medsurge floor. His medsurge nurse found him unresponsive, shallow rapid breathing, diaphoretic, and pale. When his vitals were checked, his blood pressure was 30/20. He was immediately brought to the ICU where he was intubated and received by the nurse orientee and his nurse preceptor. U
