●    Home Health—involves patients who are discharged from acute settings at a stage where they still need nursing care.

●    Hospice—palliative care of the very ill and dying with a goal of reducing stress from physical, emotional, and spiritual symptoms.

GOALS OF THE HOME CARE NURSE

●    Provide quality, cost effective healthcare in the home

●    Support, treatment, information, and understanding to patients, caregivers and families to successfully manage healthcare at home

●    Client goals include health promotion, maintenance, and restoration. By maximizing the level of independence and

self-care abilities, nurses help their client’s function at the highest possible level. In addition, nurses contribute to the prevention of complications in chronically ill persons and help minimize the effects of disability and illness.

QUALIFY UNDER MOST PAYMENT SOURCES

●    Must be homebound

●    Require skilled care

●    Require intermittent care vs continuous or acute

●    Under a physician’s care

●    Care must be reasonable and necessary
