The patient diagnosis states that she has diabetes mellitus type 1, when searching the data base, I would choose the code with the exact terms of type 1 diabetes mellitus, because all the other codes explain underlying causes or other types. This patient™s chart does not indicate underlying cause or any other type. For hypertension benign essential I input just the term hypertension and came back With 103 ICD-10 codes, 13 CPT, 8 HCPCS, 111 ICD-9 codes, and no modifiers. I would search through and again find exact terms of the written diagnosis in the patient™s chart. The list was very long using just the main term, when I put
