QI Plan Part IV HCS/588 QI Plan Part IV Davis Health Care is dedicated to providing an excellent patient care experience. A recent survey indicated that they could improve their quality of service. The survey that was conducted within the organization determined that patient safety is an area that is needed to be improved upon. Another noted area is that David Healthcare is in needed of other improvements and updates as well as concentration on patient safety. This Quality Improvement (QI) plan will describe in detail what is needed and how to use the information for making quality changes and making sure that those quality improvements succeed. To ensure that this organization keeps current with other leading organizations and the quality standards they those organizations adhere to, this QI plan will explain and help with that task. Making quality improvements at this grand of scale can be daunting and having as much information as possible will help in a smooth transition into better patients’ safety standards and quality of care. GOALS/OBJECTIVES Davis Healthcare, a local health care organization, has observed that quality standards are not up-to-date with larger leading organizations (University of Phoenix, 2016). In lieu of recent a recent survey that showed that Davis Healthcare is lacking in a couple of areas, a complete re-evaluation and organizational change is the goal. Since other leading health care organizations have successfully implemented quality improvement plans, Davis Healthcare wants to follow those organizations’ footsteps. Davis Healthcare goals are as follows: 1. Increase patient safety standards while maintaining quality care. 2. Retain and hire employees that complement these goals. 3. Create an environment of communication and success 4. Maintain a risk management program that is able to keep the organization within local, state, and federal regulations. 5. Be the healthcare provider of choice among other competitors. Data Needed to Monitor Improvements To help improve patient safety at Davis Health Care, it needs to be explained what is required to monitor improvements. Before Davis Health Care can begin to track improvements, it is important to gather data from different resources to have a broad scope and understanding of the subject before making any decisions to make improvements. The first point of data that will be needed is Risk Management. This is because the information provided here will include incidents that have occurred within Davis Health Care. The type of events and the how and when can be recorded for further analysis with data monitoring tools and applications ("Managing Data For Performance Improvement", n.d.). The incidents can include: 1. HIPPA violation, or unsecured PHI data 2. Falling risks 3. Hospital-acquired infections 4. Bed sores 5. Employees following protocol and proper patient handling The next points of evidence that will be needed are quality assurance, security department, public health department, CDC, and project management. Quality assurance plays a crucial role in data needed because QA would have the reports of grievances, diseases that are treated, fall reports, and much more. QA will also have patient satisfaction surveys to review for patient safety analysis. The security department provides general security regarding patient safety and has security measures in place to ensure the security. The public health department has information data to monitor such as alerts of community health issue or other general population health concerns. Data Collection Tools There are various types of tools that can be used to collect performance information. Three tools that are discussed are tracer studies, case studies, and administrative survey. Tracer studies, or Tracer mythology, is on site surveys that emphasize the interrelatedness of care process and standards throughout the organization (Kelly, 2011). The best usage for this collection tool is that it uses information within the organization to follow the care, treatment, and services of a determined amount of patients through the entire care process. The benefit from this is that the whole process performance can be analyzed. The next collection tool is case studies. Case studies take unique situations, or even everyday situations, and are given considerable thought to the matter. Often, the case study is a particular instance in which special attention is paid to find the underlying issue or cause. A case study is used as a training tool (" Case Study", 2016). This type of data tool is particularly useful for finding certain causes to a particular issue. Lastly, administrative surveys allow patients to give anonymous feedback on carefully selected questions. The survey can represent many problems such as facility, beds, services rendered, staff, timeliness, and so on ("Data Collection Methods", 2014).. These surveys give indicators where patient safety can be an issue and are high valuable to the organization. Tools that Measure and Display the QI data There are many different types of tools that measure and display QI data that can be gathered with the data collection tools in Part 1 of this paper. One effective tool that can be used to measure the information is a run chart. A run chart is also known as a line graph which is a graphic representation of information over a period. The run chart is perfect for monitoring performance over marked periods of time (Kelly, 2011). Another tool that can be used for measuring and displaying QI data is the Pareto chart. The Pareto chart has many advantages to including displaying data in descending order (which makes prioritizing simpler) and also displays totals using a line graph from a left to right display. Having the display in a left to right format enables the reader to understand what is most important to least important. Most important will begin on the left side of the chart. Types of Information Each Tool Measures, Displays, and reports Run Chart is best used: 1. If a manager wants to find trends and patterns in information gathered the run chart is a good solution. 2. Start-ups and short runs are easily analyzed. 3. A good way to illustrate and share information with other departments because it is easier to see patterns and trends in this format ("Run Chart", 2016). 4. Easy to construct which makes a one-time use plausible to display historical data. Pareto Chart is best used: 1.
